The Geauga County Board of Developmental Disabilities provides a School Age Coordinator for eligible children ages 3 to 14. The School Age Coordinator is here to support families by connecting them with resources, answering questions, and offering guidance based on yearly goals.

We understand that navigating available services can be challenging for families, so we offer this optional support. Many children with developmental disabilities work with multiple systems, which can be overwhelming. Our goal is to promote growth, resilience, and empower parents to advocate for their child’s unique needs.

When your child turns 14½, the coordinator will assist in transitioning them to a Transition Aged Service and Support Administrator (SSA) to ensure a smooth progression into their next phase.

 

Here’s what you can expect from your School Age Coordinator (SAC):

 

  • Service Planning: The SAC will work with your family to create a simple service plan that identifies your specific needs and concerns.
  • Professional Communication: The SAC will communicate professionally and respond to any contact from you or your family within one business day. While they may not have an immediate answer, they will acknowledge your communication and follow up.
  • Meeting Coordination: The SAC can arrange meetings with schools or other agencies involved with your family to ensure collaborative support.
  • Resource Linkage: The SAC will connect you with resources for funding, such as those for equipment or respite care, as well as other community resources as they become available.

 

Ways to Improve You Experience:

 

  • Stay in touch: Reach out to your coordinator whenever you have a need or concern.
  • Get involved: Participate in Metz functions to connect with others and stay informed.
